This form is an important legal document. It explains the risks you are assuming by participation in training with Joel Barkers and/or any member of You Reach I Teach Basketball Academy. It is important that you read and understand it completely. After you have done so, please type in your name. Typing your name in the box is the same as your signature on this document. If you are under the age of 18 it will be required to have a paper format document signed by the parent or guardian of the individual.
*I hereby give permission to You Reach I Teach Basketball Academy., its officers, employees, agents, trainers, and staff members to take whatever action is necessary for the health and welfare of my child including consenting on my behalf to any and all medical treatment, procedures, operations and/or hospitalizations and I further agree to hold them harmless and indemnify them for all medical bills incurred for the treatment of my child, and or injuries to my child.
I understand that basketball is a very physical sport and which can result in serious injury. I hold You Reach I Teach Basketball Academy., its officers, employees, agents, trainers and staff members harmless and hereby release them from liability for any injury to my child while attending any training, camps, clinics, or after school programs.
I fully understand the cancellation policy describes on You Reach I Teach's website. By filling out this form you fully grant the us permission to use for publicity & advertising purposes all photographs or video taken of the kids at the academy.
I acknowledge that I have thoroughly read this document and fully understand that it is a release of liability. By signing this document, I am waiving any right I or my successors might have to bring a legal action or assert a claim against Joel Barkers and/or You Reach I Teach Basketball Academy for your negligence or that of your employees, agents, or contractors.
