All its instructors and trainers are fully trained and accredited by a recognized National Body as required by the Insurance Underwriters. The insurance companies require all its members to have every client accept this wavier. Should you have any concerns as to whether you are able to participate please do not hesitate to contact your health professional prior to commencement. PLEASE NOTIFY US IF YOU ARE PREGNANT.

 

RELEASE AND INDEMINITY INSURANCE
TO THE STUDIO OPERATOR – THE PILATES CENTRE WA

In CONSIDERATION of the acceptance of my payment for participating in the activity (and except to the extent that the same may be precluded by statute) I AGREE TO RELEASE AND INDEMNIFY the Pilates Operator named above as
follows:

• I participate in the activity at my sole risk and responsibility;
• I release, indemnify and hold harmless the Pilates Studio – THE PILATES CENTRE WA, its servants and agents, from and against all and any actions or claims which may be made by me of on my behalf of by other parties for or in respect.

 

I also agree that if I am injured or my property is damaged, I will bring no claim, legal or otherwise, against

 

The Pilates Centre WA in respect of that injury or damage.
