Student represents and warrants to the club that student either has the permission and approval of his physician to participate in the athletic activities, programs, and exercise classes and use of exercise equipment or if he/she does not have such permission, the student hereby assumes the risk of injury and death, which may result from such activities.

I release, indemnify, and hold harmless Peter Welch's Gym Dot Ave, LLC, it's owners, operators, employees, and trainers from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Peter Welch's Gym, it's owners and operators or in route to or from any of said premises.
I herby release, discharge, and covenant not to sue Peter Welch's Gym.
