I understand that yoga includes physical movements as well as an opportunity for relaxation, stress reduction and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, discontinue the activity, and ask for support from the instructor. I will continue to breathe smoothly. I assume full responsibility for any and all damages, which may incur through participation in classes offered by Artemis Yoga, LLC.

Yoga is not a substitute for medical attention, examination, diagnosis or treatment. By signing below, I affirm that a licensed physician has verified my good health and physical condition to participate in such a fitness program. In addition, I will make the instructor aware of any medical conditions or physical limitations before class. I also affirm that I alone am responsible to decide whether to practice yoga and participation is at my own risk.

I have been made aware that from time to time, classes may be photographed or videoed by Artemis Yoga, LLC. I consent to the taking of my photograph and of being videoed and to the use, reproduction and copying of photographs and videos in connection with publicity, advertising and sales promotion by Artemis Yoga, LLC in any and all media, including on websites and print materials. I agree and understand that I will not be paid any compensation for any photographs or videos taken of me by the Artemis Parties nor do I have ownership rights in the photographs or videos taken of me.

I hereby irrevocably release, and waive any claims that I may have now or may have hereafter against Artemis Yoga, LLC and each manager, member, director, officer, employee, agent, teacher and affiliate of Artemis Yoga, LLC in connection with my participation in classes and/or based on the use, copying or reproduction of any photographs or videos taken of me.

I have read and fully understand and agree to the above liability waiver and photo release and I affirm that all of the information provided above is complete and accurate. I am signing this waiver and intake form voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law in the Commonwealth of Massachusetts.
