Hot Yoga Revolution LLC Liability Release
*YOU MUST BE 18 years old to fill out this form.
1. I am, or will be participating, in the Yoga Classes or Workshops offered by Hot Yoga Revolution (Hot Yoga Revolution LCC) in a heated room (approximately 90-105 degrees), via livestream, prerecorded (on demand) and/or outdoors, during which I will receive information and instruction about yoga and health. These classes entail intensive physical activity and exertion by me. I recognize, I am aware of and accept the risks and hazards involved.
2. I understand that it’s my responsibility to consult with a physician prior to and regarding participating in the yoga classes or workshops, and to receive prior approval to participate. I represent and warrant that I am physically fit and have no medical condition or injury, which would prevent my full and safe participation in the yoga classes or workshops.
3. In consideration of being permitted to participate in the yoga classes or workshops, I agree to assume all full responsibility for any risks, conditions, injuries, or damages known or unknown which I might incur or aggravate as a result of my participating in same.
4. In further consideration of being permitted to participate in the yoga classes or workshops, I knowingly, voluntary, and expressly waive any claim I may have or acquired against Hot Yoga Revolution (Hot Yoga Revolution LLC) for any injury, condition, or damages I may sustain as a result of entering or being on the premises or participating in the yoga classes or workshops.
5. I, my heirs or legal representatives, forever release, waive, discharge, and covenant not to sue Hot Yoga Revolution (Hot Yoga Revolution LLC), its directors, and its instructors from any and all liability, claims, demands or actions that I may have resulting from injury, death or damages arising from my participation in the yoga class, including losses caused by negligence of the released parties.
6. I understand that it is my continuing responsibility to inform the instructor(s) and staff at Hot Yoga Revolution (Hot Yoga Revolution LLC) of any previous medical condition, injuries, surgeries, prior to my fist class and at such other times as I acquire information as to the same.
7. I also understand that, except for a monetary refund I have no claims against Hot Yoga Revolution (Hot Yoga Revolution LLC) by reason of their refusal to allow me to participate in the programs.
I understand that this release discharges Hot Yoga Revolution (Hot Yoga Revolution LCC) from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Hot Yoga Revolution (Hot Yoga Revolution LCC).
I have read and understand the following In-Studio Class Policies:

• Cancellation Fee for In-Studio Classes: You must cancel no less than 3 hours before your scheduled class, or you will be charged the following:
• All Unlimited Members: $10 Cancelation fee will be automatically charged to your credit card. HYR IS NOT LIABLE FOR OVERDRAFT FEES
• Class Card/Drop-In Clients: Reserved class will be forfeited and cannot be used for a later time.
• In-studio classes will be live-streamed and recorded so that clients can also attend the practice from home or on-demand. I understand that I may appear on screen as part of the live-streaming class option.
Covid-19 Liability Waiver1. I acknowledge that I must comply with all set procedures to reduce the spread while attending all classes, required and proper use of face masks, social distancing at all times, bring and use of my own equipment.
2. I acknowledge that while Hot Yoga Revolution will take special precaution to maintain a safe and healthy environment, I understand the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Hot Yoga Revolution (Hot Yoga Revolution LCC) has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
2. I acknowledge that Hot Yoga Revolution (Hot Yoga Revolution LCC) can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions or omissions of myself and others, including, but not limited to, the studio staff, and other studio clients and their families.
3. I attest that I will not attend class if
- I am experiencing any symptom of illness (cough, shortness of breath, difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell).
- I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
- I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
- I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I understand that this release discharges Hot Yoga Revolution (Hot Yoga Revolution LCC) from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Hot Yoga Revolution (Hot Yoga Revolution LCC).
I have read the above and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
