Welcome back to the Bodytree Studio. We reopen with pleasure. We pledge that we have instituted all required governmental guidelines and we will continue to implement recommended best practices to keep you safe and healthy. In return, we ask you to acknowledge and agree to the following:
1. I understand that by visiting the Bodytree Studio and Nectar Juice Bar (“Bodytree Studio”) that there is a risk of coming into contact with the Covid-19 virus.
2. I also understand that participating in classes poses a physical risk, and I acknowledge that the use of exercise equipment involves risk of physical injury.
3. I fully understand these risks and voluntarily accept the full extent of these risks.
I hereby confirm that:
I have not been in contact with any person who has tested positive for Covid-19 in the last fourteen days;
I am not experiencing and show no symptoms of Covid-19 or any other illness;
I have not visited another country in the last fourteen days.
I hereby agree that in order to keep me and all of the Bodytree Studio employees and clients safe and healthy:
· I will comply with all of the rules imposed by the Bodytree regarding the use of the facilities and equipment.
· I will maintain social distancing rules and adhere to sanitisation procedures of the equipment that I use.
Therefore, I agree to be solely responsible for my own safety and well-being;
I accept and assume full responsibility for any illness that I contract (whether related to Covid-19 or not) and any injuries which I may experience, which may be severe and even result in death;
I release and discharge Bodytree Studio and all its employees from all liability for present or future claims, demands, rights of action, and damages resulting from or arising out of my use of, inter alia, the Bodytree Studio’s equipment, facilities, and classes. I also agree to indemnify the Bodytree Studio for the defense of and any damages resulting from any and all claims, demands, rights of action, of any person or entity, that may arise from my actions.
I also understand and agree that the Bodytree Studio is not responsible for my personal property that may be lost, stolen, or damaged while in, on, or about the premises.
I am aware that participation in a sport of physical exercise may result in accident or injury. I assume the risk connected with participation in sport or exercise and confirm that I am in good health and suffer from no physical impairment which would limit my use of Bodytree Studio.
I acknowledge that Bodytree Studio have not and will not render any medical services including medical diagnosis of my physical condition. I specifically agree that Bodytree Studio,
