I hereby acknowledge and agree to the following:
- I attest that I have no psychological, medical, or emotional condition that would prevent me from safe participation in a YYOGA class.
- Even with clear instruction there is the possibility of injury and that it is my responsibility to consult a physician regarding any and all matters involving my personal health and including without limitation:
- any prior or existing injuries; and
- my ability to participate in the programs offered before coming to a YYOGA Studio.
- I release and discharge YYOGA, its associated and affiliated companies and their respective directors, employees and instructors from all liability, claims, demands or actions that I may take resulting from injury, death, or damages arising from my participation in the physical activity classes offered by YYOGA, my utilization of YYOGA facilities and services (i.e. sauna, shower, changing rooms, bathrooms), including losses caused by the negligence of the released parties.
- I understand that YYOGA is not responsible for the loss, theft or damage of guests’ personal items.
- I understand that all YYOGA classes are provided to me for my personal use. I will not record, reproduce or share any YYOGA classes (whether in-person, through video, stream or otherwise) with others.
- Use of yyoga.ca, YYOGA at Home, and YYOGA Franchising websites are subject to YYOGA’s privacy policy and terms of use located at yyoga.ca
- I FREELY ACCEPT AND ASSUME ALL SUCH RISKS, DANGERS AND HAZARDS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE OR LOSS RESULTING THEREFROM
- I agree that:
- I am above the legal age of majority in my province or territory of residence (19 or older in BC) and do not require parent or guardian consent. OR;
- I hereby warrant and represent that I am the parent or legal guardian of the above-named minor (minors are defined as AGE 18 OR YOUNGER in British Columbia). I consent to and confirm the terms and conditions above. I hereby fully indemnify YYOGA, its associated and affiliated companies and their respective directors, shareholders, officers,agents, employees and instructors from and against any claim, action, liability, settlement, judgment, loss or damage (including but not limited to legal fees) which they may suffer or incur as a result of any death, injury or damage suffered or caused by the above- signed minor in association with his/her participation in classes at YYOGA.
- I have read this agreement and fully understand its contents and meaning, and sign it of my free will.
