I/We the parent(s) and/or legal guardian(s) of the player named below (the “Player”), having full legal responsibility for decisions regard the Player, hereby grant my/our approval for our child to participate in the hockey and off-ice training programs and other activities (the "Program” or “Programs”) conducted by Cutting Edge Hockey Ice Academy, DBA Cutting Edge Ice Academy.

I/we know that participation in hockey and off-ice training programs through the Program may result in serious and/or life threatening injuries, and in rare instances, death, and that protective equipment does not protect or safeguard against all injuries to players. In recognition of these facts, and knowing and understanding these risks, I/we hereby, on behalf of the Player, myself and our respective heirs, assigns and next of kin, freely waive all rights, release any and all claims, demands, charges, liabilities, actions, causes of action, and suits, of every name and nature, arising out of any injury of any kind, to the Player, whether the result of gross negligence, negligence or any other conduct or cause, absolve the Program Participants set forth below of any responsibility, and hereby agree to indemnify and, if necessary, agree to hold harmless the Program Participants. The term Program Participants means and includes the following: Cutting Edge Hockey DBA Cutting Edge Ice Academy, and their respective managers, members, shareholders, officers, directors, employees, volunteers, coaches, assistant coaches, supervising adults, or participating parents; FL Hockey and their respective managers, members, shareholders, officers, directors, employees, volunteers, coaches, assistant coaches, supervising adults, or participating parents; any and all volunteer organizers, coaches, assistant coaches, supervising adults or participating parents, including any game or other officials, all other participating players, and any persons transporting the Player to and/or from the Program (collectively the "Program Participants").

I/we understand and agree on behalf of the Player, myself, and our respective heirs, assigns and next of kin that execution of this document constitutes: (i) an unqualified assumption of all risks associated with the Program; (ii) a full and final release and waiver of liability of the Program Participants; (iii) an understanding not to sue the Program Participants for any loss, cost or damage arising from the Programs; and (iv) an agreement to defend, indemnify and save harmless the Program Participants from any litigation expense, legal fees, liability, damage, award or cost of any type that they may incur due to any claim made against them or any one of them whether the claim is based on the negligence or gross negligence of the Program Participants.

I/we acknowledge that there can be no reimbursement for any type of out-of-pocket expense or debt, including but not limited to, private plan insurance deductibles, transportation charges, hospital charges, physician's office charges, diagnostic tests or procedures, treatments, including but not limited to, medications, crutches, slings, bandages, counselling, etc., by any Program Participants.

I/we certify that the Player is in good physical, mental and emotional health and is not subject to any mental or physical health limitations that would prevent him/her from safely engaging in the Program. I/we grant our permission for the Program to summon emergency aid from local police, fire and other emergency services in my presence, or my absence, if deemed necessary, by any of those Program Participants or other supervising adults in attendance at the time of any incident or injury.

I/we understand that pictures or images of the Player may be taken and used for display, advertisement or other purposes. Such purposes may include but are not limited to display or advertisement on the Cutting Edge website and/or promotional literature published. By signing this agreement, I am giving the Program Participants the right to use images of the Player for any purpose without further approval from me. I am releasing all rights to any images.

I HAVE READ THIS DOCUMENT AND I UNDERSTAND THAT THE PROGRAM PARTICIPANTS ARE RELYING ON MY WARRANTIES, ASSUMPTION, WAIVER AND RELEASE WHEN ACCEPTING THE PLAYER’S PARTICIPATION IN THE PROGRAM.
