The member understands and acknowledges that there is certain risk inherent in any physical activity. Such risk includes medical/health and physical injury risk. By entering into this agreement, the member accepts and assumes those risk. If working with a personal trainer, the member also realizes and understands that a trainer is not an insurer of their safety but will do everything within his/her power to formulate a training program to the benefits of the member. The trainer is not and will not be responsible for any injury arising from the use of the programs or facilities provided by the trainer where the risk has been recognized by and acknowledged by the member.

In checking the box below, I agree that Metro Fitness is in no way responsible for the safekeeping of my personal belongings. I understand that classes at Metro Fitness may be physically strenuous, and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
