I acknowledge that that there are risks of physical injury associated with, arising out of and inherent to the activity of Ballroom dance and in recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/ or causes of action of any kind for myself or any children in my care, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Dance Kingdom, it’s officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities.

 

I understand that DK will collect and securely store some personal and billing information within the MindBody software in order to charge me for dancing that I participate in within the studio.

 

I understand I will give 24 hours notice if I need to cancel a dance lesson and will be required to pay for the lesson if I do not.

 

COVID-19  - I agree to the studio Covid-19 safety guidelines available on the website ( https://dancesportkingdom.com.au/covid-safe-summer/ ) and in-studio.  I confirm that I am double vaccinated for Covid-19.
