1. I am fully aware of the risks associated with Pilates, Yoga and other exercise programs that require strenuous physical activity; I am in good medical condition/health; I have no medical condition which would prevent me from safely participating in these activities; and I have medical clearance to participate. I knowingly and willingly assume all such risks and acknowledge that my participation in these physical activities is fully voluntary.
   
   
2. In consideration of my participation in these activities, I, on behalf of myself, my heirs, assigns, executors, administrators and representatives, hereby release and hold harmless Mindful Movement Pilates LLC, and all respective employees, instructors, contractors, agents, successor, assigns and its related “Releases”, from any and all liability, loss, damage, costs, claims and/or causes of action of every kind and nature, including but not limited to those for bodily injuries, heart complications, death, and property damage arising out of or relating to my participation in these physical activities.
   
   
3. I fully understand that I should obtain an examination and approval from my physician prior to my participation in all physical activities.
   
   

I have read this release of liability and assumption of risk agreement and fully understand its terms. I am of legal age and understand I have given up substantial rights by signing this document and sign it freely and voluntarily without any inducement. 

 

 

This is to certify that I, as parent/guardian with legal authority for this participant, do consent and agree to his/her release, as provided above, of all Releases, and, for myself, my heirs, assigns, and next of kin. I release and agree to indemnify and hold harmless the Releases from all liabilities, incidents to my minors involvement or participation in these programs as provided above, even if arising from their negligence, to the fullest extent permitted by law.
