I acknowledge that there is a risk associated with participating in fitness activities and in exercising. My participation is completely voluntary and by using Elite Fitness Downtown, LLC facility or online program, I acknowledge that I am assuming all risks of injury to myself or others including any illness or medical condition. I agree on my own behalf (and on behalf of my personal representatives, heirs, estate trustees or assigns) to release, indemnify, and discharge Elite Fitness Downtown, LLC, its owner, directors, agents, employees, or independent contractors (Elite Fitness Downtown, LLC), from any and all claims or causes of action, known or unknown, which I may have arising out of the negligence of Elite Fitness Downtown, LLC, and to indemnify and save Elite Fitness Downtown, LLC harmless from any and all claims or causes of actions, including my negligence, while at the facility or outside during an outdoor class. Elite Fitness Downtown, LLC is not responsible for any damage to, or loss of theft of my personal property. Redeeming a guest pass, signing a guest register or intake form at our fitness location will be deemed written permission and consent for future telephone contact by Elite Fitness Downtown, LLC for solicitation purposes regardless of whether or not I am listed on any Do Not Call registries.

I agree not to bring guest without the consent of Elite Fitness Downtown, LLC. All guests must have written permission from Elite Fitness Downtown LLC, and sign a Guest Waiver. If I do let a non-member into the gym without permission I understand that my membership will be revoked.

Stealing of any property inside of Elite Fitness Downtown, LLC is not allowed. Immediate termination of membership will be enforced as well as legal action to the full potential of the law.

If purchasing personal training sessions or a group class pass, I understand that the sessions and/classes are non-refundable and non-transferable unless otherwise agreed to by Elite Fitness Downtown, LLC.
Sessions and classes maybe held only if I give the trainer/ instructor 24-hour notice of my cancellation.

I agree to only use the front door on Broadway to enter Elite Fitness Downtown, LLC. I am not allowed to enter or exit through the back of the gym, only in the event of an emergency. My membership will be terminated and revoked if I am caught entering or exiting through the back doors.

 

House Rules

1. Please re-rack your weights.

2. Don't drop your weights.

3. Proper gym attire only/tennis shoes a must!

4. A towel is required at all time.

5. Wipe down your sweat from machine.

6. No gum allowed.

7. No food or drinks/water only.

8. Step outside to use your cell phone.

9. Place your towel in hamper prior to leaving

10. Have a positive attitude.
