Studio Policies & Liability Release:
In checking the box below I agree to the following policies:
- Class sign-up is required to ensure a place in each class.
- The studio permits a 12-hour cancellation window for classes and for private sessions. I understand that if I cancel my reservation after these windows, I will be subject to a late-charge and the rate of the service.
- As a courtesy to other clients, cell phones should be turned to silent, or I will notify my instructor of any emergency calls expected.
In checking the box below I agree to release any liability:
- I will notify my instructor of any new medical conditions or anything that may alter my class participation. This is to ensure my safety in class.
- I expressly assume all risks of participation in exercise provided by Conscious Movement, LLC.
