In checking the box below I agree and understand that classes at Om Yoga may be physically strenuous and I certify that I am sufficiently physically fit for such classes and have no knowledge of any health-related issues or problems that would prevent me from participating. If this is your first yoga class, Om Yoga recommends that you contact your physician before beginning to participate in yoga classes. I voluntarily participate in the classes with full knowledge that there is risk of serious personal injury, property loss or even death. I agree that Om Yoga is in no way responsible for the safekeeping of my personal belongings while attending a class. I grant permission for any photography or videography taken of me at Om Yoga or by Om Yoga or it’s vendors, to be used publicly for the purposes of marketing and advertising and/or other commercial purposes. I hereby waive, release and discharge Om Yoga, its members, instructors, employees, other clients, and staff from any and all liability, including but not limited to, personal injury, property damage or theft, or wrongful death or action of any kind, whether caused by negligence or otherwise, and agree that l, my heirs, and legal representatives will indemnify, hold harmless and promise not to sue or make any other claims of any kind whatsoever against Om Yoga, its members, employees, other clients or staff.
