Liability Release Cancellation Notice Please Give 24 hour Notice! We are nondiagnostic keepsake elective ultrasound, fitness, and massage business. You can call to reschedule any appointment and you will not be charged if reschedule or cancel within the 24 hour window notice. We know life gets in the way, so please contact us to cancel at least 24 hours before your scheduled appointment. We will charge you a late fee starting at $50.00 up to the cost of the service if you cancel less than 24 hours. Please understand that when you forget to cancel or no show your appointment without giving enough notice of at least 24 hours, we miss the opportunity to fill that appointment time, and clients on our waiting list miss the opportunity to receive services and our service providers may not get paid. Our appointments are confirmed various ways at 48 hours in advance via email or text, 24 hours in advance via text or email, and we personally attempt to call typically a day or 2 prior to the appointment because we know how easy it is to forget an appointment you may have booked weeks or months ago. Since the services are reserved for you personally, a Cancellation fee may apply. By signing this waiver your agree to the terms and authorize Pregnancy Treasures and Boutique, LLC to charge the credit card your provided if canceled within the late cancel window. If within the 24 hour window we will charge you the fee, call us to see if you qualify for a studio credit. Thank you for your understanding and cooperation.
