Contract Outline:
- 4 yoga classes per month to be used within the month
- Additional classes 25% off
- 10% member discount on workshops, special events & retail
- Four guest passes per year
- Automatic monthly payments
- No minimum commitment; two weeks' notice to cancel
- Member classes and discounts are for the member's use only
- $10 per month hold fee for vacation or illness

Contract Duration: The contract is in force for one month from the contract start date, with automatic renewal for subsequent months, unless canceled by either you or Peace of Mind Yoga, with written notification of 14 days in advance of the next billing date.
Automatic Renewal and Agreement to Pay Recurring Fees: You agree to pay Peace of Mind Yoga, Counseling, and Wellness Center monthly recurring fees via Client authorized automatic credit card or bank ACH. Payment in monthly increments by any means other than the Client's credit card or bank ACH is not permitted.
(a) I agree to purchase a membership that is an automatic charge to my credit card, or automatic debit to my checking account each month for a contract period of 1 month, with automatic renewal for subsequent months.
(b) I hereby certify that I am the holder of the credit card or an authorized signer on the bank checking account detailed below.
(c) I understand that I will be notified if my credit card or checking account payment fails to authorize for any reason and that a $10 late fee will apply if I do not provide a valid credit card or checking account ACH information within 10 calendar days of the original rejection date.
(d) I understand that my service will be deactivated if my account becomes more than 30 calendar days late.
(e) I understand that from time to time, Peace of Mind Yoga may increase this monthly fee and will notify me via in-studio signage and email with the email address that I have on file in my account with a minimum of two weeks' notice.
(f) I understand that there are no rollovers or extensions on these monthly autopay memberships. All classes must be used within the month-long membership cycle that has been pre-paid.
Cancellation:  To cancel, you must email [email protected]. Your written request to cancel your autopay membership must be received at least 14 days in advance of your billing date.
To HOLD your AutoPay: A request must be submitted in writing via email to [email protected] at least 14 days in advance of your next billing date in order to be effective for the following month. If you are placing your account on hold, you will have until the end of the contract time to use your remaining classes. There are no roll overs.

Your billing date is based on your activation date listed above. You may put your account on hold for a minimum of one month for a fee of $10/month. Following the end of your requested hold period, your monthly auto-renew will automatically be reactivated.
I understand that all memberships and discounts associated with membership are for members only and cannot be shared. No rollover or extensions on any style of membership - no exceptions. Drop-ins purchased beyond monthly allotment do not expire.
