Studio Liability Release

Our yoga teachers and massage therapists and Personal Trainers will endeavour to provide safe options at all levels however, you hold ultimate responsibility for your health at all time. Participant agrees that any pictures, audio, or visual recordings taken of him/her in connection with the class can be used for publication, promotion, articles, shows and advertisement without additional consent and without compensation at this time or any other time now and in the future.

I understand that attending Zen Life Yoga pty ltd during Non Staffed hours that i am solely and completely responsible for my safety and wellbeing and have been guided on all safety procedures before being granted access to Zen Life Yoga pty ltd, i also understand where to find the duress button and first aid equipment.

I understand that by attending Zen Life Yoga pty ltd and using the ice bath that i am solely and completely responsible for my safety and wellbeing and have been guided on all safety procedures before being granted access to Zen Life Yoga pty ltd, i also understand where to find the duress button and first aid equipment.

I understand that by attending Zen Life Yoga pty ltd and using the infrared sauna & icebath that i am solely and completely responsible for my safety and wellbeing and have been guided on all safety procedures before being granted access to Zen Life Yoga pty ltd, i also understand where to find the duress button and first aid equipment.Please be aware, that while we understand yoga / zenfit / ice bath / sauna to be your health choice, like any other physical exercise and mental practice there is some risk of injury. It is essential you take responsibility for your own practice and body during classes and sauna and ice bath to reduce this risk.

You understand that it is your responsibility to liaise with your yoga teacher / personal trainer, and inform them of any injury and that you are ultimately responsible for your body and its limitations.

By signing this waiver I understand & agree:

That I am of fit body and mind to undertake classes / sauna / ice bath and non staffed gym. I do not have any medical conditions that will impact on my ability to attend classes / sauna / ice bath and non staffed gym safely. If I am at all unsure on this point I agree it is my responsibility to consult the relevant doctor regarding this activity.

I understand and agree to my continuing responsibility to inform Zen Life Yoga / teachers / personal trainers / massage staff of changes to my health, of pregnancy, a recent surgery or anything else that may impact on my ability to exercise safely.

I understand and agree to follow the instructors direction regarding my physical safety and have sufficient awareness of my limitations to stop physical activity before it results in illness or injury.

I understand that physical adjustments are given during classes and understand it is my responsibility to let the teacher know should I not want to be 'adjusted'.

In consideration of being permitted to participate in the Activities, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the Activities at Zen Life Yoga pty ltd.

In further consideration of being permitted to participate in the Activities, I
knowingly, voluntarily and expressly waive any “Claim” (as defined below) I may have against Zen Life Yoga pty ltd, its owners, members, employees, and/or its instructors,
teachers, volunteer staff and independent contractors

I acknowledge that I have carefully read this release and waiver of liability and fully understand its contents. I voluntarily and knowingly agree to the terms and conditions stated herein.

Claim: “Claim” includes but is not limited to any and all liabilities, claims, demands, expenses, fees, legal actions, rights of actions for damages, personal injury, mental suffering and distress, or death that I may suffer, my children may suffer or that my unborn child may suffer (including any legal fees or expenses) in connection with participation in any Activity.

I accept that neither the instructor, nor the hosting facility, is liable for any injury, or damages, to person or property, resulting from the taking of the class. Those under 18 years of age must have this form signed by a parent or guardian.

I agree with the above terms.
